Job Duties:
Will assist clients with education, referrals and Medicare Part D issue resolution. Assist eligibles with applying for the low-income subsidy. Conduct education and outreach to clients, providers, family members and agencies. Prepare evaluation reports. Staff telephone hotline as needed. Experience in customer service including intake, interviews and research related to health care issues or experience in the health or mental health field. Understanding of the health care systems, laws and regulation. Training as a paralegal or experience in a law firm desired. Typical work includes but not limited to: Assist clients and faily members by assessing their Medicare Part D needs and problems and then referring, advising or representing the consumer according to established protocols and guidelines. Help eligibles with applying for the low-income subsidy. Investigate and resolve Medicare Part D issues through research; negotiate with health plans, providers, agencies and other entities as appropriate. identify those cases appropriate for further advocacy and prepare background investigation and argument. Arrange and conduct outreach activities including but not limited to presentations to client groups, professionals and community agencies throughout San Diego County. Provided input into the development of the community education and training including recommendations for outreach and changes to materials and presentations. Staff the hotline on a scheduled basis as required; respond to walk-in clients and scheduled appointments as appropriate following the established policy and procedures. Accurately enter required information on each case into databases and evaluation measures. Maintain the confidentiality (integrity) of all information. Participate in scheduled staff meetings, skill development trainings and other work-related functions as required. Upon request by the Medicare part D project CCoordinator Mental Health Program Manager, and Executive Director, perform additional duties consistent with abilities and need for the services.
